Care Needs Conversation Checklist

Use this checklist to guide a conversation with your parent, a sibling, or other family members. These questions are meant to help you gather information, not to pressure anyone into a decision.

Daily tasks and safety

  • What daily tasks are getting harder — meals, bathing, dressing, medications?
  • Has there been a recent fall, near-fall, or injury?
  • Are medications being taken correctly and on time?
  • Is the home environment safe — grab bars, lighting, clear walkways?
  • Is your parent driving? Is it still safe?
  • Are meals being prepared and eaten regularly?
  • Are utility bills being paid on time?

Social and emotional wellbeing

  • Is your parent spending time with friends, family, or community?
  • Has mood, motivation, or personality changed recently?
  • Is your parent expressing loneliness or feeling isolated?
  • Are there hobbies or activities your parent still enjoys?

Medical and health

  • What are the current diagnoses and medical conditions?
  • Who are the current doctors and specialists?
  • Are there upcoming procedures, hospitalizations, or treatments?
  • Is there a healthcare proxy or medical power of attorney in place?
  • Are there any memory or cognitive concerns?

Financial and legal

  • What is the approximate monthly budget for care?
  • Is there long-term care insurance? What does it cover?
  • Is there a will, trust, or financial power of attorney in place?
  • Who manages finances if needed?

Preferences and values

  • Where would your parent prefer to receive care — home, community, or other?
  • What matters most — independence, familiar surroundings, medical support?
  • Are there cultural, religious, or dietary needs providers should know?
  • What would make your parent feel respected during this transition?
